Jump right in and add your Name and email address.  All other information is optional.
When you enter your information you will receive an e-mail similar to the one below:

(jerry) you can make changes to your submission at any time by going to
http://www.PHS63.org/addressbook/login.asp  (The [Edit Entry] button).

You can view the Address Book and look up friends by going to
http://www.PHS63.org/addressbook/entryview.asp  (The View Address Book] button).
and clicking on the letter that corresponds to the last name.
Alternatively, you can go to http://www.PHS63.org/addressbook/search.asp
(The Search Entries] button)and search the Address Book.
The search feature will look at first, last, and nick names.
If you make a change to your entry, you will receive an e-mail similar to the one below: 

Dear Geraldine,
The information you changed for your entry has been updated.
You will retain the same ID of 16. For your records you password is
(jerry)
